Why did I get a call or text from Pathway?
Your health plan works with Pathway to help members with daily needs — things like food, housing, and rides that affect your health. The call is real. If you want to be sure, hang up and call the member line on the back of your insurance card.
Is it free? Does it affect my Medicaid eligibility?
There is no cost to you. Answering — or not answering — will not affect your ongoing Medicaid eligibility.
Who sees my information?
The Pathway team and your health plan. If a community program needs some of your information to help you, we share only what's needed and we ask you first. Some disclosures are required by law and don't depend on your permission — see our privacy notice.
Who is my navigator?
A trained member of the Pathway team — often from your own neighborhood. They give you their name, show ID at visits, and you can ask for the same navigator each time.
How do I schedule or move a visit?
Call or text your member line, or just tell your navigator. Rescheduling is never a problem — life happens.
What kind of help can I get?
Food programs, housing support, help with utility bills, rides to appointments, job and education resources, family support, and more. If we don't cover it, we'll point you to someone who does.
