Pathway to Better Days Inc. works with health plans to help members with health-related social needs. This notice explains how we handle the information you share with us.

What we collect

Contact details, the needs you tell us about, and the referrals we make for you. Health plan members are also covered by the plan's own privacy notice.

How we use it

Only to reach you back, connect you with programs, and report to your health plan as our agreements require. We never sell your information.

Who we share it with

Your health plan, and community programs you ask us to connect you with — only what they need, and only with your OK.

Your rights

You can ask what we have about you, ask us to correct it, and ask us to stop contacting you. Call or email us and we'll help.
